Internal Medicine & Pediatrics of Tampa Bay provides numerous medical services for patients for all ages. They include but are not limited to:
Triage
Answers to your non-urgent medical questions can be addressed by one of our medically trained staff members during normal business hours.
Lab
Blood draw for processing of labs ordered by our physicians. We do schedule appointments for all blood draws. Results of all labs may be called to you within 5 business days or you may be asked to schedule an appointment to discuss the results. If you are given a lab slip and labs were done elsewhere, you may be required to schedule an office visit to discuss results with your physician.
Referrals and Radiology Test Authorizations
Referral service provided during business hours for processing all paperwork required by insurance companies for specialist or diagnostic services. Please note that the normal processing time is 3-5 business days. Without sufficient notice you may be asked to reschedule your referral appointment.
Prescription Refills
Service provided during business hours for refilling medications prescribed by our physicians. To help expedite this service, please give us your pharmacy phone number or have your pharmacy fax any refill requests directly to our office. Allow 72 business hours for refill requests to be completed. Refill requests must be made during regular business hours. Refills will not be filled by the on-call physician after hours, weekends and Holidays
Website
We provide you with important information, directions and patient forms on our website. You can print out your new patient forms ahead of time and bring them with you to your first visit. In addition, informative medical related websites are available on our "Links Page" in our website.
Medical Records
Our medical records department will request your medical records from your previous physician, however, you must sign an “Authorization to Release Medical Records” allowing us to receive these records. This form can be downloaded on our website or requested at our front desk. Any records requested to be copied for personal use will have a charge of - $1.00 per page up to 25 pages then $.25 per page.
HIPAA
Confidentiality is important to you as a patient and we are committed to following all HIPAA guidelines. Records, test results, health and other personal information cannot be released to anyone besides the patient or guardian without prior written authorization. If you wish to authorize us to release or discuss your health information with someone other than yourself or guardian, please complete the “Consent for Release of Medical Information” section of our new patient form.
Living Will
This documents your directives as to life support. If you have executed a Living Will please bring a copy so we may place this in your medical record.
Co-Pays
The patient portion due for physician services per insurance contract. This amount will be due at time of service, we accept Visa, MasterCard, check or cash. A $25.00 service fee will be charged to your account for all bounced checks.
Insurance Submission
As a courtesy we will submit most insurance claims and bill the insured for any non-covered or patient portion due. Payment is due upon receipt of the bill and questions can be directed to the billing department in our office by calling 813-422-6316
Website
FMLA forms require a completion charge of $20.00 per form or an optional office visit to complete the form with the physician. This charge covers the time involved for the physician to review necessary information and complete these forms. Please note: We do NOT fill out Short or Long Term Disability forms or Workman’s Comp forms. Also School forms and sports physical forms are exempt from any charge or fee.
After Hours Service
Our doctors provide 24 hour on-call coverage for urgent medical issues. Non-urgent issues and prescription refills will not be addressed by the on-call physician.
"Med-Peds" is an abbreviation for the combination of both Internal Medicine and Pediatrics. A Med-Peds Doctor refers to the physician who has trained in both specialties of Internal Medicine and Pediatrics.
Internal Medicine and Pediatrics offers downloadable forms for its patients to fill out at their own convenience. Download and fill out the neccesary forms before you visit to decrease your waiting time.
